Comme tous les êtres vivants, le virus SARS-CoV-2 évolue. Au cours de chaque infection sont produits des milliards de nouvelles particules virales. Parmi ces nouveaux virus, certains sont porteurs de mutations.
Ce processus d’évolution et cette génération de mutants ont été étudiés en détail. On sait aujourd’hui que dans la majorité des cas, ces virus SARS-CoV-2 mutants sont soit non transmis à de nouveaux hôtes, soit « neutres », c’est-à-dire que les infections qu’ils causent sont semblables à celles causées par les virus non mutants (aussi appelé « sauvages » ou « historiques »).
Mais parfois certains de ces mutants se propagent et prennent le pas sur les virus existants. Les exemples les plus récents ont pour noms variants Alpha, Bêta, Gamma, et maintenant Delta. Ils ont émergé pour la première fois respectivement au Royaume-Uni, en Afrique du Sud, au Brésil ou en Inde. Pourquoi ? Que sait-on de leurs propriétés ? Que sait-on de leurs capacités à échapper à la vaccination ?

Des fois que les gens ne mourraient pas en masse avant, des fois que les écoles n’étaient pas des lieux de diffusion du virus, des fois que…
Mais zut.
La première « vague » a fait 30 000 morts (dont une bonne partie étaient évitables : confinement trop tard, absence de masques, etc.).
La seconde vague, alors qu’on était parvenus à quasiment éradiquer le virus au début de l’été, et malgré une bien meilleure connaissance des modes de transmission, malgré la très large disponibilité des masques, et sans doute de meilleures compétences pour traiter les gens, a fait 65 000 morts. Avec un gouvernement qui a passé son temps à claironner qu’il avait vachement mieux géré cette deuxième vague que la première…
En fait, à la sortie du « deuxième confinement » (15 décembre), on totalisait près de 60 000 morts. Depuis cette date, il y a encore eu 37 000 morts. Une « normalité » totalement assumée qui a fait plus de morts que lors de la première vague.
Tout cela avant que les variants ne « changent la donne ».
Alors à chaque fois que je lis ou entends cette histoire de variants qui changent la donne, je trouve que c’est à la fois d’une connerie et d’une indécence totales.

Aux #états-unis, alors même que de l’#argent_public à été massivement injecté, la #FDA a laissé les #laboratoires_pharmaceutiques définir les critères d’efficacité de leur(s) propre(s) #vaccin(s) contre le #SARS_Cov2 ; par exemple chez Pfizer il suffira d’avoir significativement moins de #COVID-19 non graves que dans le bras placebo pour conclure à l’efficacité du vaccin ..., avec, de plus, comme corollaire la possibilité de pouvoir arrêter leur essai lors des résultats intérimaires et d’homologuer le vaccin. quitte à voir apparaître des effets secondaires graves par la suite.
Par ailleurs il n’existe aucun essai comparant les candidats vaccins entre eux, pour le plus grand bonheur des labos bien sûr..
L’#OMS a bien prévu des essais rigoureux en #Europe avec des comparaisons entre produits, mais les choses ont à peine démarré..

Yet that only happened in one case—the government-run trial of the antiviral drug remdesivir. It didn’t happen with any other therapeutics, and hasn’t happened with vaccines. Instead, the regulatory agencies let the pharmaceutical companies define the terms of their own trials. “It makes me bonkers that we think we should let the companies decide on the study designs, because their incentives are off,” Bach says. “When we know definitively that X or Y are not what we want, and we want something slightly different, that’s where the government is supposed to step in and modify the market’s behavior.”

We evaluate the contribution of presymptomatic and asymptomatic transmission based on recent individual-level data regarding infectiousness prior to symptom onset and the asymptomatic proportion among all infections. We found that the majority of incidences may be attributable to silent transmission from a combination of the presymptomatic stage and asymptomatic infections. Consequently, even if all symptomatic cases are isolated, a vast outbreak may nonetheless unfold. We further quantified the effect of isolating silent infections in addition to symptomatic cases, finding that over one-third of silent infections must be isolated to suppress a future outbreak below 1% of the population. Our results indicate that symptom-based isolation must be supplemented by rapid #contact tracing and testing that identifies asymptomatic and presymptomatic cases, in order to safely lift current restrictions and minimize the risk of resurgence.

In a study of 77 well characterised infector-infectee pairs in Hong Kong, it was estimated that the serial interval (duration between symptom onset of a primary case to symptom onset of its secondary case) of COVID-19 was 5.8 days (mean), with 7.6% of serial intervals distributed negatively (i.e. the infectee developed symptoms prior to infector), strongly implying pre-symptomatic transmission [He X, Lau EH, Wu P, et al. Temporal dynamics in viral shedding and transmissibility of COVID-19. Nature Med 2020; 26:672-675]. Assuming a median incubation period of 5.2 days (based on other studies), the study estimated that the infectious period of SARS-CoV-2 started 2.3 days before onset of symptoms, peaking at 0.7 days, and declining within 7 days.

Local data, based on an analysis of 766 patients, indicate that by day 15 from onset of illness, 30% of all #COVID-19 patients are PCR-negative by nasopharyngeal swab, this rises to 68% by day 21 and 88% by day 28 and by day 33, 95% of all patients are negative by PCR (NCID data). While the duration of viral shedding1 by PCR may extend to a month and sometimes longer for a small group of patients, and several jurisdictions including Singapore have been using it to guide de-isolation and discharge policies, it is important to note that viral RNA detection by PCR does not equate to infectiousness or viable virus.
A surrogate marker of ‘viral load’ with PCR is the cycle threshold value (Ct). A low Ct value indicates a high viral RNA amount, and vice versa. As noted above, detection of viral RNA does not necessarily mean the presence of infectious or viable virus. In a local study from a multicenter cohort of 73 COVID-19 patients, when the Ct value was 30 or higher (i.e. when viral load is low), no viable virus (based on being able to culture the virus) has been found . In addition, virus could not be isolated or cultured after day 11 of illness . These data corroborate the epidemiologic data and indicate that while viral RNA detection may persist in some patients, such persistent RNA detection represent non-viable virus and such patients are non-infectious.

Robust data are lacking regarding infectiousness of asymptomatic and pre- symptomatic individuals and how much asymptomatic infection drives transmission. However, asymptomatic persons may have similar viral shedding patterns[13]. A Singapore study found pre-symptomatic transmission of SARS-CoV-2 in ~6.4% of 157 locally acquired cases, but no asymptomatic transmission [Wei WE, Li Z, Chiew CJ, et al. Pre-symptomatic transmission of SARS-CoV-2 - Singapore, January 23 - March 16, 2020. MMWR Morb Mortal Wkly Rep. 2020; 69: 411- 415].
Summary and Conclusion
Based on the accumulated data since the start of the COVID-19 pandemic, the infectious period of SARS-CoV-2 in symptomatic individuals may begin around 2 days before the onset of symptoms, and persists for about 7 - 10 days after the onset of symptoms. Active viral replication drops quickly after the first week, and viable virus was not found after the second week of illness despite the persistence of PCR detection of RNA. These findings are supported by epidemiologic, microbiologic and clinical data. These new findings allow for revised discharge criteria based on the data on the time course of infectiousness rather than the absence of RNA detection by PCR testing, taking into consideration both the clinical and public health perspectives, including the individual patient’s physical and mental well-being. In addition, given these findings, resources can focus on testing persons with acute respiratory symptoms and suspected COVID-19 in early presentation, allowing timelier public health intervention and containment.
